or addition in a contact person for your hospital Compassionate Use Risk Factor Identification - (Updated Apr 2008) - Identifies the criteria and definitions for variables to use to determine if an individual case should be designated as compassionate use. Compassionate Use Spreadsheet - Updated form includes validation checks on some of the fields. Please include this form to submit or update compassionate use cases for each quarter. Transfer/Discharge Spreadsheet - This spreadsheet is to be used for submitting all transfer and discharge hospital information on all patients, with the September 2006 procedure dates. This must be submitted with each quarterly submission. Instructions for using this spreadsheet can be found in the TransferDischargeInstructions.doc document.
