Mass-DAC Policy - Policy regarding data submissions to Mass-DAC

Demographic Data Format 2.61 - (Updated 7 Apr 2008) - Instructions for formatting demographic file fields based on STS 2.61 specifications. If your software only allows you to enter a 2-character value in the COUNTRY field, than you should use the 2-character country abbreviations from the ISO web site, ISO.org 3166 Country Code Lists. If you can enter the full country name, that is preferred.

Database Submission Verification Form - Include this form with each data submission that you send to Mass-DAC
Data File Sign Off Form - Used after final error report corrections have been returned to Mass-DAC in the error corrections spreadsheet.
Change in Contact Form - Send this form when there is a change/deletion or addition in a contact person for your hospital
