Submission Instructions:

Demographic File Specs (XML - ACC 4) - (Updated June 24, 2009) Added details on how to submit the Mass-DAC demographic as a delimited text file or as an XML file. The zip file contains the summary document for all extra fields required by Mass-DAC, the XML Schema definition for the Massachusetts demographic file, a sample XML data file, and a list of fields dropped from the version 3 file (no longer required in the version 4 file). These are based on the ACC 4.3 specifications.

Data Submission Verification Form - Include this form with each data submission that you send to Mass-DAC

Change in Contact Form - E-mail or fax this form to the Mass-DAC Project Manager Ann Lovett when there is a change in one of the roles for personnel working with Mass-DAC at your hospital.

Hospital Forms (Non-Mass-DAC):

Lahey Clinic Cardiology Cath Lab Data Form - This is an optional form for hospitals to use for data collection based on ACC 3.0 specifications. Do not send to Mass-DAC.
