All Massachusetts hospitals that perform angioplasty or percutaneous coronary interventions (PCI) must submit patient and physician specific data to Mass-DAC on a quarterly basis. Data will be collected by each hospital utilizing the American College of Cardiology (ACC) National Cardiovascular Data Registry (NCDR) Instrument. All data will be collected with no exclusions, including identifying demographic information on both patient and physician. Data Submission Verification Form - (Updated September, 2011) Include this form with each data submission that you send to Mass-DAC. The updated form now reports the Exceptional Risk cases independent of the Compassionate Use cases, and a new Aortic Stenosis case line has been added.

Mass-DAC Transfer Hospital Numbers - Data collection field list of hospital numbers used to identify transfers from and discharges to another acute care facility.
Data Submission Demographic File Specs - Provides details on how to structure the Mass-DAC demographic data as a delimited text file or as an XML file. The zip file contains the summary document for all extra fields required by Mass-DAC, the XML Schema definition for the Massachusetts demographic file, a sample XML data file, and a list of fields dropped from the version 3 file (no longer required in the version 4 file). These are based on the ACC 4.3 specifications.
